How the numbers were produced.

Forty questions were written in five intent groups and frozen before the first run. They are reproduced in full above.
- 02
Each question was sent 3 times to each instrumented engine with web search available and a neutral system instruction, so the answers reflect what a patient would see rather than a prompt engineered to name practices.
- 03
Every completed answer was passed to a separate, closed-book extraction model that lists the practices and named individuals in the text. Those strings were matched to a published alias table, and any link to a practice's own domain also counts as a citation.
- 04
A practice counts as present in a run if it was named in the text or cited by link. Present runs divided by completed runs is its Answer Share. Runs that named nobody are reported, not discarded.
- 05
Independence and ownership flags are drawn from public sources and the NPI registry census of San Diego fertility locations. They are labels on public entities, not claims about quality.
- 06
Engine outages, missing keys, and un-instrumented surfaces are reported as coverage gaps. The denominator is never reduced to make a line rise.

Because only one was funded for this sweep. The other four are listed as coverage gaps rather than removed from the page, which is the same rule we apply to client reporting: an engine that was not swept is shown as missing, never quietly dropped from the denominator.
